How do I get a work permit request form?

You can apply for a work permit after you have been hired by an employer.

 

Obtaining a work permit consists of the following:

 

  1. Download and complete form B1-1 found on this page. This form requires parent and employer signatures.
    PLEASE NOTE: STUDENT ADDRESS MUST MATCH THE ADDRESS THE SCHOOL HAS ON FILE.
     
  2. Return the form to the Career Center (H119) COMPLETE with parent and employer signatures.
    • Completed work permits forms can be walked into the Career Center Room H119 to be processed OR scanned and emailed to Kristine Danao.
    • Please allow a minimum of 2-5 days to process.
